Your insurance provider will continue to pay laboratory testing  fee schedules to your laboratory provider.  Origin Concierge Phlebotomy Services only charges a convenience fee to the patient. 

We accept payment via debit and all major credit cards. To confirm an appointment all fees must be paid on the day scheduled appointment is made.

We strongly suggest scheduling as far in advance as possible if your schedule is busy and you have a specific date and time you would like a phlebotomist to arrive. Please note: Morning appointments are the most frequently requested.  Call +1 (800) 957-8593 to schedule.

To schedule an appointment please  call +1 (800) 957-8593 to schedule. Pleases have a copy of your doctor's lab orders faxed to +1(833) 542-3024 no later than 24-48 hours prior to your scheduled appointment. This is to insure all equipment will be prepared and at appointment location. Please have a clear copy of your insurance cards (front and back) and driver's license on the day of appointment. This information is only used for specimen laboratory delivery.  Please note that all paths of communication are in HIPPA compliance and kept private and confidential.

 Appointment cancellations can disrupt our scheduling and inconvenience our customers scheduled times. To ensure the smooth operation of our mobile phlebotomy services, we have the following cancellation policy: 

For cancellations made within 24 hours of your scheduled appointment time, the full price of the service will be charged. We require a 24-hour notice to reschedule or cancel your appointment without incurring additional charges.  We understand that unexpected situations may arise, and this fee helps us cover some of the costs associated with scheduling and preparation for your appointment. 

We strive to provide the highest quality of service, and your cooperation with these policies allows us to continue offering convenient and efficient mobile phlebotomy services.

We understand that unexpected situations may arise.  We  try to provide a 10-minute grace period for all appointments if possible but, this  is not guaranteed.  After the grace period is reached the appointment is considered a no-show and therefore canceled. We strive to provide the highest quality of service, and your cooperation with these policies allows us to continue offering convenient and efficient mobile phlebotomy services to all clients. 

The turnaround time for most routine tests is 2-3 business days, however some specialty tests can take up to 7 business days. Please consult with your physician directly regarding your results.

Please note: Origin Concierge Phlebotomy Services does not receive a copy of your test results.
